The State of Men's Mental Health: A Global Review with a Focus on Kenya
·
Text size
Mental health is a critical component of health that affects one's thinking, feeling, decision making, stress management, and social relationships. Mental health is defined by the World Health Organisation (WHO) as a state of well-being that allows a person to realise their capacities, cope with the everyday stresses of life, work effectively and participate in their communities. Although mental disorders are significant, they are a large public health problem with more than 300 million individuals in the world affected.
Men's Mental Health Awareness Month, which is celebrated in June, focuses on raising awareness of the distinct mental health concerns of men and promotes discussions about mental health. While men and women are equally vulnerable to mental health issues, societal expectations, cultural norms, and traditional perceptions of masculinity can make it less likely for men to seek professional assistance or show signs of emotional distress. This can lead to many men suffering from untreated mental health issues that can contribute to depression, anxiety, substance abuse, marriage problems, loss of productivity, and in some cases, to suicide.
Mental illnesses remain a major burden on individuals, families and health systems worldwide. Despite a growing awareness of mental health problems in men in recent years, there is still a great deal of stigma around seeking timely care for mental health issues. This challenge is also noted in Kenya where factors such as poverty, cultural norms, unemployment, family obligations, and lack of mental health services are seen to add to the burden of mental illness among men.
The article discusses the state of men's mental health on a global level and in Kenya, including the impact of mental health issues on men, and initiatives underway to raise awareness and increase access to mental health care in Kenya. The discussion highlights the need to raise awareness about men's mental health and create an environment that allows men to access mental health services without stigma or discrimination.
Global overview of men's mental health
Mental health disorders contribute to the majority of disability and poor health globally and impact on individuals with all ages and social backgrounds. Before the COVID-19 pandemic, the World Health Organization (WHO) estimated that almost 1 billion people globally suffered with a mental disorder, which represents a significant burden of mental illness worldwide (WHO, 2022). Mental health issues can impact both men and women, but because there are societal expectations that men are supposed to be emotionally strong, self-reliant, and resilient, they may be more susceptible to experiencing challenges. These norms are a disincentive for many men to seek help for emotional distress or go for a professional psychological assessment and treatment, leading to a delay in detection and treatment. The common mental health issues prevalent among men are depression, anxiety disorders, substance use disorders, and suicidal behaviour. Men are found to be less likely to seek mental health service than women, even when they are highly distressed psychologically. Rather, some men may deal with their feelings by engaging in destructive routines, including overindulging in alcohol, drug abuse, aggression, or social seclusion, that may worsen their mental well being and result in early death. The promotion of mental health among men has thus emerged as a public health concern worldwide, and a call for more awareness, early detection, intervention and availability of mental health services to men has been called for by governments, health care providers and non-governmental organisations.
Abstract
Education is one of the key factors of a child's future opportunities and social mobility. Although Kenya has been implementing various educational reforms for decades to increase access to quality education, socioeconomic and geographical factors continue to play a major role in determining access in the country. This article puts forward the view that poverty, gender differences and unequal distribution of education resources are responsible for the perpetuation of educational inequalities in Kenya. The study builds on Karl Marx's Conflict Theory and explores how current social and economic inequalities persist and stifle the educational opportunities and outcomes of less privileged learners. The article examines some of the challenges faced, including low infrastructure, low teacher numbers, unequal funding, and high drop-out rates among children from low-income families, using secondary data sourced from government reports, international organizations and scholarly literature. The study shows that while education reforms have led to greater enrollment, structural socio-economic inequalities remain to be a threat to equal access to quality education in Kenya.
Global statistics of men's mental health
There is increasing evidence of the global burden of mental illnesses. In 2019, the global average of people living with a mental disorder is estimated to be around 970 million, or 1 in every 8 people on the planet. Depression is an estimated 280 million people and anxiety disorders affect an estimated 301 million people worldwide (WHO ,2022). Suicide is one of the most serious outcomes of mental illness that is not treated. The WHO estimates that over 720,000 individuals commit suicide annually, and that in most nations, suicide deaths are mainly among males. Men also are much less likely to access professional mental health care, which increases the risk that mental health issues will not be managed until they have reached a high level of severity.
Moreover, mental disorders are one of the top ten causes of disability globally, representing a substantial proportion of the Global Burden of Disease (GBD) years lived with disability (YLDs). This has a significant economic cost as well, as depression and anxiety are thought to cost the world economy nearly US$1 trillion per year in health care costs, absenteeism and lost productivity. These statistics show that mental health is not just a matter for individuals, but a huge issue within public health and socio-economic policy which needs coordinated and concerted efforts to combat stigma, promote access to mental health services and promote help-seeking amongst men.
Men's Mental health concern in Kenya
Although poor men's mental health is a globally public health concern, it's impact varies across the world due differences in socio-economic, cultural, health system and access to mental health services differences across the country that influence the impact of such issues. Mental health is under- resourced, under-researched, and under-resourced in many low- and middle-income countries, including Kenya, where its increasing burden is not being met. Men are often discouraged from seeking psychological support or expressing psychological distress by cultural expectations that encourage them to be strong, emotionally resilient, and self-reliant. For this reason, a lot of Kenyan men experience depression, anxiety, drug abuse and, in some cases, commit suicide without anyone realizing.
Over the past few years, there has been increased awareness of mental health as a public health problem in Kenya. There are, however, still some obstacles to overcome, such as the lack of mental health facilities, a lack of specialized facilities, stigma and the socioeconomic pressures of unemployment, poverty, and family responsibilities. Understanding men's mental health in the Kenyan context is therefore important to appreciate the problem in its particular context and develop interventions that can address mental health issues among Kenyan men.
Mental health has become an increasingly important public health issue in Kenya, with evidence indicating that many men experience mental health disorders but often do not seek professional help. According to the Kenya Mental Health Policy (2015–2030), mental disorders contribute significantly to the country's disease burden, yet access to quality mental health services remains limited. The World Health Organization estimates that approximately 25% of outpatients and up to 40% of inpatients in Kenyan health facilities have mental health conditions, demonstrating the widespread nature of the problem.
Among Kenyan men, depression, anxiety disorders, alcohol and substance use disorders, and suicide are some of the most prevalent mental health challenges. Traditional gender norms that associate masculinity with strength, emotional control, and self-reliance often discourage men from discussing their emotional struggles or seeking counselling. As a result, many men suffer in silence until their conditions become severe.
Socioeconomic factors further increase men's vulnerability to mental health problems. High rates of unemployment, financial instability, poverty, work-related stress, and the expectation of being the primary family provider place considerable psychological pressure on many Kenyan men. Relationship difficulties, family conflicts, and bereavement also contribute to emotional distress. In addition, alcohol and substance misuse, particularly among young men, frequently coexist with mental health disorders and may worsen depression, anxiety, and suicidal behaviour.
Suicide remains a major concern in Kenya, with men accounting for the majority of reported suicide deaths. Studies have consistently shown that Kenyan men are more likely than women to die by suicide, highlighting the urgent need for targeted mental health interventions and early support services. Despite increasing public awareness, stigma surrounding mental illness continues to prevent many men from accessing available mental health services.
Efforts to improve men's mental health in Kenya
Although the Government of Kenya has made progress through the Kenya Mental Health Policy (2015–2030) and the Mental Health Amendment Act, mental health services remain unevenly distributed, with shortages of psychiatrists, psychologists, and counsellors, particularly in rural and underserved areas. Strengthening community-based mental health services, promoting public awareness, and encouraging men to seek professional support are essential steps toward improving mental health outcomes in Kenya.
The Government of Kenya and other stakeholders have stepped up their efforts in building the capacity of mental health services and addressing mental illness stigma. These are programs that will be offered to the general public, but they are especially relevant to men who are less likely to seek professional mental health help because of cultural norms and societal expectations. The Ministry of Health has developed one of the key interventions, namely the Kenya Mental Health Policy 2015-2030. Policy is a national framework to support good mental health, to prevent mental illness, to include mental health in primary health care, to ensure better access to good quality mental health services and to reduce the discrimination of people with mental illness. The policy also highlights the need to promote community-based care and public awareness as key components in improving mental health outcomes.
The Ministry of Health has rolled out the Kenya Mental Health Action Plan (2021-2025) to implement the policy. The Action Plan aims to decentralize mental health services through integration in primary healthcare facilities in all counties, boost capacity of the mental health workforce, enhance public education and awareness, and improve coordination between the national and county governments. The plan includes mental health as a key aspect of Universal Health Coverage (UHC). In 2024, the Ministry of Health released the first Clinical Guidelines for the Management of Common Mental Disorders in Kenya, in addition to an adapted version of the WHO Mental Health Gap Action Programme (mhGAP). These guidelines are designed to enhance the identification, diagnosis, treatment and referral of mental health conditions in primary health care. The Ministry says about 42% of individuals accessing primary health care suffered from severe depression and 10.3% of the Kenyan population have common mental disorders. The Ministry said the programs are designed to improve the country's estimated 75% mental health treatment gap by training frontline health care professionals in the treatment of common mental illnesses.
The Government has also set up the National Taskforce on Mental Health to inform the reforms in mental health governance, mental health financing, increase of number of trained mental health professionals and promotion of mental health community awareness. Mental health is also a priority area in the Kenyan Universal Health Coverage programme which calls for the enhancement of equitable access to mental health services in Kenya. In addition to government initiatives, the National Authority for the Campaign Against Alcohol and Drug Abuse (NACADA) is a big player in promoting mental health among men by organizing public awareness initiatives on substance use and mental health. Throughout Men's Mental Health Awareness Month, in June each year, NACADA empowers men to seek assistance and breaks down barriers to openness and honesty about depression, anxiety, and suicide prevention, while also dispelling negative stereotypes about men and crying.
There have also been several non-governmental and community-based organisations (NGOs and CBOs) which have helped to make a difference to the mental health of men in Kenya by offering counselling, psychosocial support, advocacy and community education to the community. They operate in tandem with government institutions to alleviate stigma, promote early help-seeking and increase access to mental health services, including for vulnerable populations. While mental health provision in Kenya has made significant strides, sustained investment in mental health infrastructure, public awareness and mental health education for mental health workers is needed to combat the increasing burden of mental illness among Kenyan men.
Recommendations
Addressing men's mental health requires coordinated efforts from governments, healthcare providers, communities, employers, educational institutions, and families. The following recommendations are essential for improving men's mental health globally and in Kenya Governments should increase investment in mental health by allocating adequate financial and human resources to expand mental health services, particularly at the primary healthcare level. This will improve access to affordable and timely mental healthcare for men in both urban and rural areas.
Public awareness campaigns should be strengthened throughout the year, not only during Men's Mental Health Awareness Month in June. These campaigns should challenge harmful stereotypes that discourage men from expressing emotions or seeking professional help and should promote positive masculinity and emotional well-being. Community-based mental health programmes should be expanded to provide accessible counselling, peer support groups, and psychosocial services. Engaging community leaders, religious institutions, and local organizations can help reduce stigma and encourage men to seek support at an early stage.
Workplaces should implement comprehensive mental health policies that include employee assistance programmes, stress management initiatives, mental health education, and confidential counselling services. Since many men spend a significant portion of their lives at work, employers have a critical role in promoting psychological well-being. Schools and universities should incorporate mental health education into their curricula to equip boys and young men with emotional coping skills, resilience, and knowledge of available mental health services. Early intervention can prevent mental health problems from progressing into adulthood.
More research should be conducted on men's mental health in Kenya to generate reliable data on prevalence, risk factors, help-seeking behaviours, and the effectiveness of interventions. Evidence-based research will support the development of policies and programmes that respond to the specific mental health needs of Kenyan men.
Conclusion
Men's mental health is a critical public health issue that affects individuals, families, communities, and national development. Globally, millions of men experience mental health disorders such as depression, anxiety, substance use disorders, and suicidal behaviour, yet many remain undiagnosed or untreated because of stigma and deeply rooted societal expectations regarding masculinity. The consequences extend beyond the individual, contributing to family instability, reduced workplace productivity, increased healthcare costs, and preventable deaths.
In Kenya, the burden of men's mental health continues to grow amid economic challenges, cultural expectations, and limited access to quality mental health services. Although the Government of Kenya and various stakeholders have made significant progress through policy reforms, awareness campaigns, and the integration of mental health into primary healthcare, substantial gaps remain in service delivery, workforce capacity, and public awareness. Improving men's mental health requires sustained commitment from government agencies, healthcare professionals, employers, educational institutions, civil society organizations, communities, and men themselves. By reducing stigma, promoting early intervention, expanding access to quality mental healthcare, and encouraging open conversations about mental well-being, Kenya can make significant progress toward ensuring that men receive the support they need. Ultimately, investing in men's mental health is not only an investment in individual well-being but also in stronger families, healthier communities, and sustainable national development.
